Why do I keep abandoning productivity apps?

Most apps ask you to maintain a structure — tags, projects, priorities, reviews. That maintenance is executive-function work, which is the resource that's already stretched. When the maintenance lapses the system stops being trustworthy, and an untrusted system gets abandoned.

Does an app help with task paralysis?

It can help with one specific part: the deciding. If the freeze comes from too many undifferentiated options, having something else produce a ranked order removes that decision. It doesn't address the other causes, and it isn't a treatment.
